Clinical Support Specialist

Lumina Hospice and Palliative Care founded as Benton Hospice Service in 1980, is a community-grown nonprofit, that provides compassionate care and support for individuals, families and caregivers facing serious illness or end of life.

Lumina Hospice & Palliative Care (Lumina) is seeking a full-time Clinical Support Specialist to join our team. As the Clinical Support Specialist your primary responsibility will be supporting the administrative and operational needs of the clinical team to achieve the Lumina mission of providing compassionate, patient-centered care at the end of life. The Clinical Support Specialist coordinates referral activities for Hospice and Palliative Care programs and ensures clinical documentation is complete, accurate, timely, and in compliance with applicable State/Federal laws, regulations, and Lumina policies. Your responsibilities will be to:

  • Provides dependable and accurate support in the admissions process in multiple areas while interacting with patients, families, referral sources, co-workers, and vendors. Provides direct assistance to admissions nurse.
  • Enters and processes referrals in electronic health record and obtains required documentation from patients’ attending provider and Hospice Medical Director within required timeframes.
  • Coordinates medical equipment and supplies needed by patients at the time of admission.
  • Works closely with clinical staff to ensure all required admission, discharge/death and transfer documentation is complete in the required timeframes.
  • Reviews charts and prepares electronic documentation for required care plan updates prior to each patients’ certification and re-certification period(s).
  • Prepares electronic documentation for weekly Interdisciplinary Team (IDT) Meeting; reviews chart and ensures clinical staff has completed required care plan updates.
  • Under direction of a nurse, social worker, or physician, promptly communicates patient needs to staff, pharmacies, durable medical equipment suppliers, patients’ referring/following provider and the Lumina Medical Director.
  • Processes Hospice Item Set questionnaire and submits required data to Medicare for each patient admission and discharge within the required timeframe.
  • Scans, files, and organizes paper documentation in the patient’s legal medical record.
  • Provides direct administrative support to Housecalls program.
  • Accepts and navigates referrals and onboarding of new Housecalls patients.
  • Verify insurance eligibility and preauthorization when needed.
  • Reviews authorizations for HIPAA compliance and releases the minimum necessary to fulfill valid and lawful record requests within the requested timeframe.
  • Reviews patients’ charts for completion and closes records within 28 days after discharge from hospice.
  • Maintains the security and integrity of patient, caregiver, bereaved, community partner, referring provider and other data by adhering to Lumina data management policies and work instructions.
  • Answers phones and greets visitors with professional courtesy, referring calls and escalating to the appropriate staff when needed.
  • Provides cross-coverage to members of the team as needed.
  • Trains volunteers on department support tasks while maintaining ultimate accountability for the quality and timeliness of their work.
  • Serves as assigned on agency-wide initiatives and/or committees.
  • Participates in QAPI activities as requested.
  • Contributes to communication, positive work environment, and team effort.
  • Understands and adheres to Lumina Privacy Policy; Lumina Abuse of Vulnerable Persons Policy; and all policies and procedures contained in the Lumina Employee Handbook.
  • Performs other duties as assigned.

Qualifications:

  • Commitment to serving the needs of the terminally ill.
  • Bachelor’s degree in business, communications, project management, health care, computer science, or related field OR combination of equivalent work experience and education preferred.
  • Proficient in Microsoft Office Suite, Adobe, and SharePoint. Excellent data entry skills.
  • Ability to learn electronic heath records software.
  • Willingness to stay current on changing hospice professional and regulatory requirements as related to clinical documentation.
  • Familiarity with medical terminology and ICD 10 coding preferred.
  • Excellent oral and written communication skills.
  • Strong organizational and planning skills.
  • Demonstrated attention to detail and accuracy in work assignments.
  • Customer service experience preferred.
  • Bilingual Spanish speaking preferred.

Schedule: Usual hours fall within the 8:00-5:00, Monday through Friday.

*Preferred applicant will be asked to pass a background check and drug screen following a job offer.
