Social Worker, Hospice Per Diem/Casual

Lumina Hospice and Palliative Care founded as Benton Hospice Service in 1980, is a community-grown nonprofit, that provides compassionate care and support for individuals, families and caregivers facing serious illness or end of life.

Lumina Hospice & Palliative Care in Corvallis, Oregon is seeking a Social Worker, per diem/casual, to join their team. As a Social Worker you’ll get the opportunity to work with programs and services designed to honor choice and dignity, while providing comprehensive patient and family care. This role collaborates with the patient, family, hospice team, and other care providers to identify and address psychosocial and practical needs related to the terminal illness.

Your responsibilities will be to:

  • Assists with admission procedures, including obtaining a psychosocial assessment
  • Assess, evaluate and regularly reevaluate needs of the patient and family, organizing resources to meet those needs.
  • Provide counseling to the patient and caregivers related to end-of-life issues and education on grief issues and coping skills
  • Educate patient and caregivers on grief issues and coping skill.
  • Assist and support patient and family in utilizing relevant community resources, making choices that best meet their needs, and coordinates social services with other community agencies
  • Develop and implement a plan of care for patient/family/ and/or care givers in consultation with other interdisciplinary group members (IDG) while bringing psychosocial, spiritual, physical and financial conditions affecting the patient and family to team conferences.
  • Document all patient/family contacts and interventions and maintains accurate and up-to-date patient records and care plan.
  • Participate in QAPI activities
  • Assist with the Bereavement program; referrals to Lumina and/or community resources, and communicating in the community about the Lumina Mission, Vision and Values as needed
  • Provide care in a variety of settings, including homes with unpredictable environments such as uneven surfaces and potential hazards such as animals, smoke, and exposure to communicable diseases etc.
  • Maintains the standards of Social Work excellence in accordance with principles set forth by the profession, NASW Code of Ethics, and the National Hospice and Palliative Care Organization

Qualifications:

  • Requires a Masters of Social Work from an accredited School of Social Work and possess a current Oregon Social Work license.
  • Prefer two years supervised social work experience that includes individual, family and/or group counseling, with preference for experience in hospice, palliative care, or health related field
  • Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes
  • Demonstrated skill in communication; patient documentation requirements, and service coordination with the ability to work successfully with patients and family members in aspects of terminal care
  • Intermediate skills in email, word processing, internet, keyboarding, and ability to learn to use medical software to complete accurate and timely charting
  • Must be reliable and have the ability to work independently and as a member of a team in delivering assistance within scope of practice
  • Bi-lingual, Spanish speaking ability is helpful

Schedule: Hours are varied and may fall within the 8-5, Monday through Friday work week although occasional other hours may be required to meet patient needs and fluctuating workloads as needed.
